The short version

In 2021 the FDA cleared Deep TMS for anxious depression — depression accompanied by significant anxiety symptoms. This matters in practice because anxiety historically predicted a poorer response to antidepressants, and many patients were told TMS was not for them.

Anxiety on its own, without a depressive episode, is not an FDA-cleared TMS indication, and no reputable New Jersey clinic will tell you otherwise. If a clinic advertises TMS as a standalone anxiety treatment and does not discuss a depression diagnosis, treat that as a warning sign.

What a good New Jersey provider will do is assess both symptom sets at intake, use a protocol appropriate to anxious depression, and track anxiety scores alongside depression scores through the course.

What a course of treatment looks like

    Consultation and eligibility

    A psychiatrist reviews your diagnosis, medication history and safety screening, then confirms whether TMS is appropriate.

    Insurance authorisation

    The clinic submits your medication trials and therapy history to your insurer. Approval usually takes one to three weeks.

    Mapping session

    Your first visit finds the right coil position and the lowest energy setting that works for you. It takes about an hour.

    Daily treatment

    Weekday sessions for six to nine weeks. You sit in a chair, stay awake, and drive yourself home afterwards.

  5. 5

    Taper and review

    The final sessions are spaced out while your prescriber reviews your scores and plans maintenance if you need it.

Eligibility

Is TMS likely to be an option for you?

  • Major depressive disorder diagnosis with significant anxiety symptoms
  • Tried at least two antidepressants without adequate relief
  • No ferromagnetic metal or implanted device in or near the head
  • No history of seizures or an untreated seizure risk

Questions people ask

I have anxiety, not depression. Can I have TMS?

The clearance covers depression with anxiety. If anxiety is your only diagnosis, a clinic may still discuss off-label options, but insurance is unlikely to cover it.

Will TMS make my anxiety worse at first?

Some people feel more activated in the first week. Clinics can adjust intensity and timing, and it typically settles.
