What TMS costs in New Jersey and how insurers approve it

Nearly every major New Jersey plan covers TMS for depression. The hold-up is almost never coverage — it is documentation.

Every major insurer operating in New Jersey covers TMS for major depressive disorder, but all of them require documented failure of prior treatment first. Horizon Blue Cross Blue Shield of New Jersey, the state's largest carrier, generally requires trials of at least four antidepressants from two different classes plus a course of psychotherapy. Aetna and Cigna typically require two to four failed medication trials. Medicare covers TMS in New Jersey under Novitas Solutions' local coverage determination, which requires trials of four medications or documented intolerance. NJ FamilyCare (Medicaid) coverage exists but is limited and prior authorization is strict. Most New Jersey clinics handle the authorization paperwork for you — ask before your consultation.

Covers TMS for major depressive disorder with prior authorization. Typically requires documented failure of two or more antidepressants at adequate dose and duration, plus a trial of psychotherapy.

Aetna

Covers standard rTMS and, for many plans, Deep TMS. Expects documentation of medication trials and a current depression rating scale score.

Cigna

Covers TMS for treatment-resistant depression. Requires prior authorization and re-authorization partway through the course.

UnitedHealthcare / Oxford

Covers TMS under its behavioral health benefit. Authorization is usually handled by Optum rather than the medical plan.

Medicare (including NJ Advantage plans)

Covers TMS for major depressive disorder. Advantage plans add their own prior-authorization step.

NJ FamilyCare / Medicaid

Coverage varies by managed-care organization. Fewer New Jersey clinics accept it, so ask before scheduling an evaluation.

Getting approved faster

  • Bring a list of every antidepressant you have tried, with doses and how long you took them.
  • Ask your prescriber to note the reason each medication was stopped — side effects count.
  • Have a recent PHQ-9 or similar score on file; most insurers want a current measure.
  • Let the clinic submit the authorization — their staff do this daily and know the templates.
  • If you are denied, appeal. Denials for incomplete documentation are routinely overturned.
