Formal Complaint Regarding Psychiatric Nurse Practitioner Encounter To Whom It May Concern, I am writing to file a formal complaint regarding my psychiatric appointment today at Harmony Bay Wellness, located at 17-15 Maple Avenue, Fair Lawn, NJ. My appointment was with a nurse practitioner identified to me only as “Michael P.” I am a foreign-trained physician and currently a medical student in my final year in the United States. I have a documented history of ADHD and depression, with prior evaluations, screenings, and diagnoses confirming ADHD. I recently moved from New York to Jersey City, which is why I sought to establish psychiatric care with a new provider. During the visit, I explained my medical and psychiatric history, including my previous ADHD diagnosis and prior evaluations. However, the provider stated that she “does not believe in psychiatric screening.” This was very concerning to me, because psychiatric screening tools and clinical evaluations are part of standard psychiatric assessment. When I asked how she makes psychiatric diagnoses if she does not consider screening or prior evaluations, the interaction became tense. I also explained that I was previously treated for ADHD and asked about Adderall as one possible treatment option. The provider stated that because I asked about Adderall, she felt uncomfortable prescribing it. I clarified that I was not demanding a specific medication and that I was open to any treatment she believed would be appropriate. I simply wanted my ADHD symptoms to be addressed appropriately, along with my depression. The provider then said she wanted to treat my depression first and that depression medication may help my ADHD symptoms. I told her that I was open to that plan, but I also asked what would be done specifically for my ADHD. She repeatedly stated that she did not like the way I was speaking to her. I explained that I was trying to be honest and open as a patient, especially because psychiatric care requires trust and clear communication. The encounter escalated further when the provider told me to leave her office. I felt dismissed, disrespected, and denied a proper psychiatric evaluation. I was not given a clear treatment plan, and I was not provided with a professional explanation of my options. I also asked for the provider’s full name, but she told me to look for it myself in the chart. When I asked the front desk for her full name, I was told they could only provide “Michael P.” I am very concerned about the professionalism of this encounter, the provider’s statement that she does not believe in psychiatric screening, and the way I was treated when discussing my documented ADHD history and treatment needs. I believe patients should be able to discuss symptoms, prior diagnoses, and treatment options openly without being accused of inappropriate behavior or being removed from the visit. I am requesting that this matter be reviewed. I would also like the full name and credentials of the provider I saw today, documentation of the encounter, and guidance on how I can continue care with a different qualified psychiatric provider. Would never return to this location. Due to the street of the practice undergoing construction, I was required to take a detour that made me 9 minutes late to my appointment and Karen refused to see me. I have had this appointment for several months and despite the construction and difficulties I had finding the office (hidden in the back of a separate building) she made no exception. I will never return to Karen and would never recommend her to anyone who is seeking any mental health care as there is no care for the patients.
